Is FLR(IED) and Tier 2 are same? No.

Is UKBA issued FLR(IED) for new applicants to switch the category from TIer 1? No.

Is UKBA issued Tier 2 General for 5 years? No.

Please advice, or was there any mistake by UKBA Case worker? Perhaps not, you would need to elaborate on your circumstances.
