Change of address and issued with new passport

I have tier-2 work permit with dependents. I wish to inform UKBA of the following:

1. Issued with new passport, old passport has expired
2. Chang of address

My queries are:
1. Do I fill the form for each dependent and myself:

2. Do I need to send the BRPs along with form?
