ILR - Final check(and a few questions)

1>PEO date check(is it valid?)
Purely based on Tier1, get the initial Tier1 inside UK.

Visa history:
------------------------------------------
Initial Tier1(3 years):
Date of Issue: 14 APR/AVR 08
Valid until : 10 APR/AVR 11
------------------------------------------
Tier1 Extention(2 years):
Date of Issue: 08/03/11
Valid until : 10/04/13
------------------------------------------
PEO date:
Tuesday, 19 March 2013
Is this date ok based on my Visa history(5 years and within 28 days) ?

Print out of PEO appointment letter
2 passport photo with full name at back
current passport
Current Tier1 Biometric card
ILR application form
12 month bank statement(Feb/2012 - Feb/2013)
12 month payslips(Feb/2012 - Feb/2013)
Police Registration Certificate
Life in the UK PASS Certificate
Employer letter stating my basic information(base salary, position etc)
No educational supporting evidence as I provided on Tier
Tier1 extension confirmation letter from Home Office.
Previous 4 year's P60 letters(year 2013's is not available yet)
A print out showing I am not out of UK for over 180 in the past 5 years( March/2008 to March/2013).

Are those listed above enough?

3>A few more questions:
3.1>In the earning section 9
Because of bonus and overtime I did , my 12 months salary(Feb/2012 - Feb/2013) added up is a bit more than my annual salary which is stated in my employer letter.
Which amount should I claim for ? The annual salary provided by HR or the actual earning (including bonus and overtime) ?

N.B either amount would not change my claiming band because overtime and bonus is not much.

3.2>In section 11 - Documents
There are so many options but as a Tier1(General) holder, it seems I only need to check "11L" apart from the checkbox for "A Life in the UK test pass notification letter". Is this correct ?

3.3> Section 9 C - Age
I should claim based on the age when I first granted Tier1 in year 2008 , NOT my current age. Is this correct ?

Any input is appreciated.

N.B either amount would not change my claiming band because overtime and bonus is not much.
