eea4 application help for my parents

hi,
i am sending application for eea4 for my father and mother. and want to know what documents require to send with the application.

here is history
I got my eea4 pr and my wife got eea3 pr (german national)
my parents currently got RC for 5 years which they already complected living with us.

now we sending for my parents eea4 application so just wondering what documents do we need to send???

i am thinking to send following documents.
my passport
wife passport
both pr for myself and wife.
marriage certificate.
my birth certificate.
my proof of residence for past 5 years.
wife proof of residence for past 5 years.
father passport.
mother passport.
2 pictures of each.
father proof of residency for past 5 years.
mother proof of residency for past 5 years.

let me know if these are ok or anything else we need to include.

do we still need to send our proof of residency for past 5 years??? as they already seen them. my wife had her pr on march 2013 and i got my pr on april 2013
