Section 1: 1.1 Previous UKBA ref. No, Can you advise which reference number need to fill?
Section 2:2.1 Date of first arrival, I came to UK for student visa 27/1/2004 and then work permit 13/6/2006. Which dates need to fill?
2.2 Last 5 years absences only?
2.3 Main home is UK or England?

Section 3: 3.1 Occupation – I am working as contractor and director of limited company. Can I fill Director or E&P Design Engineer (title of present job?)
3.2 Can I tick both Director and self-employed or which one?
3.3 employer name: my ltd company? Or agency name?
3.4 Above?
3.5 I recently started ltd company (Feb 2013) since then I am paid VAT return and NIC PAYE. I haven’t done any self-assessment. What are the documents to need to support application form for Naturalisation?

Section 5 Referees: My 1st referee is “University Friend” and director of Ltd Company (not British citizen) and 2nd referee is “Ex-colleague” Electrical Engineer and British Citizen.
The above referees are OK to proceed my application?

The most recent HM Revenue & Customs Self Assessment Statement of Account. If not what you have and explain that you have not received the required document yet as your company if newly started.

Providing they meet the other requirements - have known you for at least 3 years etc. that should be fine.
