Urgent help tommorow going to NCS

Hi

Thanks a lot for all my previous questions answered by experts.
I have last few questions pls answer.

1) Only one photo is enough ?
2) Photo to be stick or staple in a small plastic cover on referee page.
3) I didn't filled AN form with pen rather filled electronically and printed with referee pages also. I have only taken signatures on the form as rest was computerised and printed. Is it OK.
4) I have no income as a Director I just put employee on form. or to tick both.
5) I would like to give cheque pls let me know whose name should I Write.

Employee and director of ltd which one?

I checked the guide thanks for your help
Pls one more question

I m working part time and also a director of my ltd company.
I have only income as an employee.
Do I need to tick both as an employee & as a director
Or only employee is ok. Will it effect my application
if I don't mention As a director.
Pls advice.

Why I m confused because I got p60 from employer
Where it shown my last year income.
But as a director I had p60 from my accountant which is NIL.
Pls help me thanks
