Refusal - Refusal form or online application

dear friends,

UKBA hasnt sent me an appeal form but has given me a right to appeal. Please can someone clear following points?

1. Where to get an appeal form? (any web link please)

2. I heard but not sure that you can apply for appeal online on UKBA website.

3. Can we fax the form -- if yes then please can i have a fax number.
