MN1 application regarding

Hello,

I have an appointment with NCS on 13th June, 2013 for my kid's MN1. I have few questions,

- There is no payment details page on the application, How will I be paying the payment? Is it only by cheque?

- Referees, can they be family friends? Nursery workers? my colleagues who know my kid?

- My kid was born here in UK, so would that mean the required documents are his birth cert, passport and our marriage cert. Am I missing any?
