When Can I Apply FLR (M)

Hi,

I will be applying my PR (SET O) on July 29th 2013 (Direct appointment) and my wife came as dependent in Jan 2013 so i have to apply FLR (M) for her.

Can anyone let know when can I apply for my Wife FLR(m) appointment,If i can apply the next day through direct appointment, do I need my Visa card,as I will be only having approval letter on next day.
