FLR(M) Refusal

Please I need help regarding my application.
My case goes thus
I applied for leave to remain in December 2012,but I received a refusal notice yesterday on the ground that I have not meet the English language requirement.
However,during the time of the application I have already achieved a Uk bachelors degree but I for got to send my certificate with the application.
Please I have been given right to appeal within 10 working days but I don't know what to write on the appeal form.
Could someone help me out please on what to write and which documents to send.Thanks
