Tier 2 Dependency to Tisa 2 General

Hi, hoping somebody can provide some advice....

(1) Can somebody on a Tier 2 Dependency visa apply from within the UK, or must they go back to their country of origin, to apply for the Tier 2 (General) Visa?

(2) If they can apply from WITHIN the UK, should they be given an UNRESTRICTED or RESTRICTED COS?
