Urgent query please respond/Tier 4 extension

I have filled a print and send tier 4 application form but i just need to clarify something:

I have my dependent with me so do I need to fill her separate form?


UKBA has charged me total fees and they have asked all the information about my dependent in online form but I was just confuse.

I remember when I had an extension in 2011 they use to ask everybody to fill separate form.
