Queries regarding Proof of Earnings

Hello,

I am in the process of completing my application for the HSMP, and have a couple of queries I was hoping someone could answer for me please.

Although I am a New Zealand citizen, I have been working in the US.
In order to prove my income I must apply for exceptional circumstances as the Income Tax Return I am able to provide, (the W-2 Form, (Wage and Tax Statement), which is a summary of income earned and taxes paid for the previous calendar year,) does not cover my entire earning period, which extends into 2007.

1. The application form asks me to prove why I am unable to provide a W-2 for my entire earning period. Is it enough to state that US law dictates employers must provide their employees with a W-2 for the previous tax year, by the end of January, and that there is no provision for receiving an off-cycle W-2? Or do I need my employer to document and sign a letter stating this?

2. I would like to provide bank statements to prove my 2007 earnings; however I am unable to provide original copies as I receive all of my Bank Statements online. I visited my bank and asked them to print out my statements, and stamp and sign them to verify they were true and correct. They did this, however, when printed, the statements were 3 pages long, and although the bank stapled all three pages together, they only stamped and signed the front page of each statement.
Do I need to have every page stamped and signed or will it be sufficient to have the statement stamped and signed on the front page only please?
