Tier 2(General) Dependant Visa Form Questions

Hi,

I'm in UK under Tier 2(General) Visa (Sponsored by Company)

I wish to bring my Wife with me from PAKISTAN, on dependant visa.

But When I read the "Application form for Tier 1, 2 and 5 dependants of points-based system" from UKBA Website

under Section A, they didn't mentioned "Tier 2 (General)" can anyone please guide me which one my wife should have to tick?

Or

does she have to Apply somewhere ONLINE?
