Change of Name & extension of FLR(M)

Hi

I would really appreciate some help with my application:

These are my circumstances

1. Currently hold spouse visa (granted in Nov 2013) expiring in July 2016
2. The spouse visa was issued in my maiden name and is a "visa sticker" in my passport
3. Applied for New Passport with marriage certificate as evidence and issued new passport in my married name from the Indian High Commission in the UK.
4. The New passport states that I have a VALID VISA in my previous passport. I carry both passports when travelling and have never had any issues at all.

Question is: Do I need to fill in the change of circumstances form as I do not have a BRP. It is also too late to do this as I need to apply for the extension to my FLR(M) in July 2016!!

Any advice would be graciously received!!
