ILR for Tier-1 General Dependents

I came to UK as Tier-1 Gen in Jun 2010 and got my ILR in Feb 2016.
My wife and Kids joined me in UK in Aug 2011 and living with me since then.
Now I am planning to apply for their ILR(since I didn't do it along with me due to some reason).

I am not sure which application form I should use? Is it SET (O) or SET(M)? And DO I need to show my last 12 months earning again? And do I need any support letter in any specific format?
