ILR after 6 Year Discretionary Leave SET(O) Form

I would like to apply for settlement visa (ILR) but i am confuse when i will be eligible to apply. I was first granted Discretionary Leave to remain on the 17th August, 2010 - 16th August 2013 (3 years). The second Discretionary Leave to remain was granted on the 12th October, 2013 which will expire on 12th October, 2016. I am planning to use premium visa service because the university (Nursing) have asked me to provided ILR by 24th August. What document do i need to take. (Planning to book premium visa service on 18th August 2016). What form do i need to use to apply for ILR? After i completed my fir 3 years dlr on the 16th august 2013, i tried to applied for ilr but the HO said i need to complete 6 years in DLR.that is the reason i was delay and on granting 2nd visa (dlr) top of it, they took another 1 months to granted dlr.Could you pleae answer these question. thank you.

Make a good cover letter to explain you are applying for settlement after 6 years DLR

I advise you to use premium service centre too.

Include all originals such as your previous DLR grant letters and your cohabitation docs, your payslips, bank statements etc
