Visitor Visa and free NHS treatment

Hi,

I am in the UK since one year on a Long Term T2 ICT VISA and working in a reputed MNC.
My spouse had visited me on a 6 Month Multi entry Visitor VISA from India from November 2015 to March 2016.
During her stay here as a visitor, she had to visit a local A&E department (upon the recommendation from a local Walk-in center) as she had developed an inflammation on her finger tip.
The doctor had to perform a small incision to relieve the finger off the infection and had also prescribed an antibiotic which we had bought and paid for from a local pharmacist.
The A&E department did not charge us for their service.

Now my spouse has to apply for a Dependent VISA under the same category as my VISA. The VISA application has a section as below:

"Have you ever received medical treatment in the UK?"
Did you pay for your treatment

As I understand, my wife should answer YES to the first question but I am not sure about the second question as we did not have to pay for her treatment (Although we had paid for the prescribed medicine).

Would there be a case of any concern for the application if my wife answers the second question as a NO?

Would it make a difference to mention the exact case in the additional details section of the application?

Responses are highly appreciated as I am finding myself in a difficult situation as I have read people having mixed opinions on similar situations.

Immigration Officer making a note

Hi,

I am on a Long Term T2 ICT work permit in the UK since last year.
My spouse had visited me in November last year, second time on a new Visitor VISA from India, after having visited for 3 months previously in the year 2014 on her first Visitor VISA.

Although she had mentioned a period of 20 days for the visit in her Cover Letter while submitting her application, she had received a 6 Month Multi entry Visitor VISA.

Due to this, we planned to extend her stay to 4 months in the UK, because of which she had to resign from her job in India.

After landing in the UK, the Immigration Officer at the airport noticed her long intention to visit for 4 months (as she had showed her February 2016 return flight ticket to the officer) and asked her why did not she apply for a dependent VISA instead.

As she could give a satisfactory reply, the officer told her that he would allow her to pass through but also said that he would be noting down the situation in the system.

My wife left the country about 1 week before expiry of her VISA.

Now she is applying for a Dependent VISA under the same category as mine and we are worried about any reason for the application to get rejected based on the notes made by the immigration officer on her last visit as a visitor.

Has anyone had a similar experience of immigration officer having noting down your situation in their system and any subsequent applications having taken longer than normal or getting rejected based on your previous visits?

Your responses would be highly appreciated.

Did she pay NHS rates or the full cost of her medication?

She paid the full cost of medication in the pharmacy.
