Application form for ILR under Tier 2 Dependent

Hi

I am now under Tier 2 and My wife and 12-year-olds daughter are under T2 dependent.
I am applying for ILR with my family at the same time. (Tier 2 > ILR).

I wonder if dependent visa holders need a separate application form or I can add my family details to set O.

Could you advise me? Which would be grateful.
