SU07/12 FORM

Hello Everyone,
I have British passport, and my wife has dependent visa until Feb 2017. We had child in April 2016 in India. I would like to bring my child here. Now there is 2 page form called SU07/12. Who would certify the 2nd page? Please respond. I have sent all the paperwork below:
1.My &My wife passport copy 2.Birth certificate of my son. 3.Passport of my son 4.Marriage certificate 5.My travel tickets to India 6.Lease of my Flat 7.Council tax letter 8.Gas & Electricity bill 9.Letter from Employment. 10.P60 11. 3xmonths pay slips.

please advice me about filling up of 2nd page of SU07/12 form. I hope I have submitted all paper work required for my son's visa.

My Agent back in India has asked me to fill this form. Can you please give your form to ask some more questions if it is possible.

Assume your child is not British?
