ILR for dependent child only

I have ILR which i got on 5/10/2015. my wife got ILR on 4/4/2016. i have a daughter who is 8 years old was born abroad. currently she is on tier 2 dependent visa which is expiring on 13/09/2016. i have to apply for ILR for her so i need to know which section of the form (form SET(O) i have to fill in.

like section 1 , should i put my details or my daughter details her.
in section 2 whom detail should i put in ??
should i leave blank section 4 as it is about knowlege of language and life in uk
similarly section 5 about home and detail. should i put my details

and do i have to send my origional ILR and my wife ILR with application
also do i have to send my job detail, pay slip marriage certicate ???

Hi, what form did your wife fill for her ILR? If she has filled Set (O), it should follow the same patern as they are both dependants.

Your child is the applicant, you put her details in the form.
