form to fill in at PEO centre

Hello everyone..I am planning to apply for my ILR(LR) at peo centre...I am currently on T1 (E)50k year 2

I was going through the previous posts ..most of the people are saying that they have been given a form to fill in at PEO centre and it takes only two minute to fill in ...all I know is this is something about your tax history.could anyone tell me what this form is all about..and what specific information it asks for ?
If anyone has got template of the form, it will be highly appreciated

1. During your residence in the UK, have you worked in a self-employed capacity and has been registered as self employed or worked as the director or share-holder of a limited company?
2. What is/was the nature of your business as a self-employment person (select all that apply)

3. Please confirm the financial years in which you were registered as self-employed in the UK?

9. Are you satisfied that the SA tax returns submitted to HMRC accurately reflected your self-employed income?

Strange isn't, that you now log in and post a reply using the first ID created on 26th August and not the one created on 13th September.
