Tier 2 application submission after work start date

Good day
I have applied for a tier 2 general U.K. visa (applying outside of U.K.)
Visa application date (when payment was made): 19/7/16
CoS expires on 15/10/16
Start date on CoS: 1/10/16
Actual start date, as agreed by sponsor: 7th or 14th November 2016

Please advise when I should make my appointment at visa centre to submit application
> before 1/10/16(start date on CoS) or after?
> if before, when is the latest that I can book
