Main Applicant has ILR, spouse apply for Tier 2 extension

Hi,

My wife who is the main applicant has got her ILR recently with a validity of 5years on it. My Visa is tier 2 dependent visa, expiring on Feb 2017. I'm still not eligible for ILR (3 more years to go), therefore I have got to apply for my extension.

Can you guide me on which Visa Application form should I complete for this process?
Do any of us (the main applicant and me) need to have show bank statements and payslips? if so, how many months and how much?

Both main applicant and me are employed.
Do I need to produce a letter showing my working status or my wife's (who is the main applicant).

Basically, I would like know, what are documents we have to produce?

Can you tell me, if I (dependent) or my wife(ILR) need to hold some money in the bank for certain months for the application process.
