EEA national as a main applicant:

01) Frist fill the Payment details with EEA national details
02) Biometric : fill with Non-EEA national details
03) Section01 :
A. EEA national
B. EEA national details
C. Non-EEA national details
04) Section02: No need to fill
05) Section03: 3.1 second box tick
06) Section03: 3.2 First box tick
07) Section05: Two copies; one for EEA national and one for Non-EEA national
08) Section09: EEA national activities only
10) Section11: Non-EEA national details and relationship with EEA national
11) Section16: EEA and Non-EEA national public fund details (No need to add Meternity pay)
12) Section17: Based on EEAn and Non-EEA national
13) Section18: Proof of document for each section
14) Section19: A. EEA national main applicant declaration and Ignore B

My husband - Bulgarian ( EEA national, applying as a main applicant), I'm (NON-EEA national).
We've got stuck filling in SECTION 5. We've managed to fill in his copy, but not sure about mine 5.6 where they ask 'Were any of the above documents issued to you on the basis that you are, or were, the family member or extended family member of the sponsor named in section 2?
My husband holds a Registration Certificate, and I've been previously issued with a Residence Card based on his registration certificate. Also we did not fill in SECTION 2. And now we're confused what shall we say in 5.6?

The form is poorly-designed and poorly laid out.
It has to apply to a lot of different scenarios.

Suggest state something like: "I obtained my RC as the direct family member dependent of main applicant who is my sponsor".
The caseworker does this every day, she will figure it out. That's why you're paying the £65.
