Help to fill MN1 form

Hi dear all!
i am going to apply for kids "child registration as a BC"by using MN1 form
please someone help to answer Q 1.3 and Q 1.5 ON Page 3:
1.3-please say if and when the child was given
indefinite leave to enter/remain in the UK.
if the child's parents is an EEA national,or
swiss national or a family member of an EEA
or Swiss national you should ensure that they
qualify for permanent residence.
child has ilr to enter/remain in the UK D D/M M/ Y Y Y Y
As the above 1.3 does not apply on my kids who born in uk and i hold ILR status if in am not wrong.
1.5-surname/family name(note:the name you given
here will be the name shown on the child's certificate
so please ensure it is spelt correctly and you have
written it in the correct order)
As the above 1.5 it asks only last name which i written down his last name only or i have to include full name.
please any seniors could u explain me what is best answer as i provide information.if you need further information please ask me,will be highly appreciated

1.5) Surname only
