ILR Document Checklist For Tier 2 General

Hi All,

I will be eligible for ILR in December, I am in Tier 2 General at this moment. Initially when I came to UK I was on Tier 1, searched for job and I got it, after 6 months my firm transferred my visa to tier 2 General, so in total 5 years, I was on Tier 1 for 6 months and last 4.5 years in Tier 2, now my doubt is where do I need to update this option in Set (0) form, is there any template to fill the set (0) form.

Can any one provide any Employer letter template where I can submit my to my firm.
