Tier 1 50k extension-- points required

Hello,

i am applying for extension on this Friday and i need some help.

Question# 1

Registered with HM Revenue & Customs (HMRC) as self-employed;

or
 registered with Companies House as a director of a new or an existing business. Directors who are on the list of disqualified directors provided byCompanies House will not be awarded points.

20 points for this.

current appointment report will be enough for this or do i need some thing else for this?

On a date no earlier than 3 months prior to the date of application, you were:

current appointment report again for these 15 points?

These are B and C from the points table. i am not sure about these. please advise me that current appointment report is the right document for B and C?

(1) current appointment report
(2) CT41G OR CT600 that has got your company UTR number
