Questions regarding to EEA (FM) form.

Hello & thank you for your advice.

I am EU8 national with DCPA issued 24 Jan 2017

I am now looking to apply for registration certificate for my wife and daughter (born 02.07.2015)

Obviously i know that I am in this form as a sponsor and my wife name is on this application.

Could you advice my what sections shall I fill up.

Do I need to send any ID/Passport of my daughter or just born certificate?
