Extension of Leave to Remain

Discretionary Leave Extension

My cousin brother was granted discretionary leave to remain in UK and it will expire in start of next month.
He was granted leave to remain because he has a child contact order to see his children.His wife is from India and he has two children.Wife claimed Asylum on domestic violence and she was granted 5 years asylum status in UK.
My cousin brother was given a child contact order from family court and on the basis of that order he was granted leave to remain in uk.He continuously see his children every weekend and maintained his contact He is working full time and both children are in full time education in UK.

Questions

1- Which visa application form he has to use?

2- What type of evidence he will need to get further leave to remain?

3- Any further documents he required ?

4- Any English test he needs to take?

Do he needs to use Form FLR(FP) or Form FLR-HRO?
