Quick question regarding 'sign declaration' online form

Hi all,
I have just completed filling online form (General partner) on my wife's behalf. She will be joining me here in UK.
I came accross the 'Sign Declaration' section.

Which is like this
Signature
Date Signed

So is it my wife's name or my name on that signature box? and its mandatory field.
