To fill the Table 3b2, In the Policy Guidance and
Apllication form they asked to write the Hourly Pay rate whenever it is changed.
One of my employee started in Dec'15, minimum pay rate at that time was £7 then in April'16 it changed to £7.20 and after that I changed the pay rate to £8.

Can you please tell me how should I fill the table as there are these columns:

Employee Name Start Date End Date Hourly Rate

Where should I mentioned the Date? The start date is the date when employee start working with the company.

Start date when the employee started.
End date is the date before the salary changed.
Next row start date is the date when salary changed and so on
