ILR premium service appointments 2017

Hi everyone

I am trying to book my appointment (premium service) for my ILR. On the home office site, it reads the following for Tier 2 holders:
Tier 2 or 4 visa Book as part of your online application

However, when you go to the SET_O application form and guidelines, there are no links to fill an application online. I need to make an appointment at the Croydon office before the last two weeks of October 2017 . I read a post from 2014 that the Home Office withrew the SET_O online application from their site and an email was given to make the premium service appointment. Anyone knows whether this may be the case again?
I will really appreciate if someone could help me with this.
