Documents for Tier 4 dependant (Child) visa

Hi everyone!
I wish to apply for a Tier 4 Student dependant visa for my child born in UK. My wife and I are Pakistani national. Can anyone share their recent experience on application such documents required for the Tier 4 dependant (child) visa born in UK? Further, do they need to pay IHS as a part of application?
