Tier 2 Dependent visa extension application

Hi,

I have UK Independent Leave to Remain which was obtained through 5 year residency route on Tier-2 general visa. I need to now apply for my wife's visa extension, who currently has PBS dependent leave to remain.

One of the questions asked in the form is 'Sponsor license number'. I do not understand this as there is no company sponsorship involved. Has anyone else encountered this? Let me also know if I am filling a wrong application form.
