Switch from Tier 2 to ILR dependant

Hi,

I am currently holding ILR. My wife has a Tier 2 visa but she would like to switch to my dependant.

1. Is it possible for her to switch from Tier 2 to ILR dependant within UK?
2. Can she apply in person at a premium service centre?
3. Is FLR(M) the right form to use?
