FLR (M) - Application Form Query

Hi Friends,

I am currently on ILR and applying for my wife's first and children's extension (she is currently on dependant/spouse visa) by end of this month. I was looking at the FLR (M) application form and it ask us to disclose public funds (page 37 of application form). My understanding is, if i claim child benefit on my name as i have got ILR, it will not affect my wife's visa extension

I have got few questions, if anybody can clarify please:

1) is FLR (M) correct application form for extension?
2) Do i need to disclose that i am claiming child benefit on the application form?
3) I am trying to calculate the nhs health surcharge and not sure which option should i select for questions "What is your visa route" and "What is your visa type"? (I am aware that it is 500 pounds for each dependant however i am not sure what option to select for above 2 questions)

1. Yes, if she is on a spouse settlement visa and not a PBS dependent visa.

2. Yes.

3. What options are you given on the online form?? Note that we cannot see what the online form asks.
