naturalisation as self-employed: accountant letter template

I am ILR and will apply for naturalisation as self-employed in November 2018. I started working as self-employed in May 2018, before was employed, and therefore have not filed a tax return. I am registered as self-assessment.

A solicitor indicated on the phone that I need to attach a letter from my accountant with my income/taxes during the last year, both from self-employment and employment, among other information. My accountant has asked me if there is a template for the letter or what specifically needs to be included in it. Anyone knows? Also, is there any other information from accountants (self-employment) that needs to be included in addition to the letter?
