I am at the moment filling up Appendix 1 form and have 2 doubts
regarding Maintainence section (Originals Proof) and Earnings section
Doubt -- 1
7.3 Section (Maintainence Funds)
Put a cross (x) in the relevant boxes to indicate what evidence of these funds has been supplied. All documents must be original
----- Its said "All documents must be Original"
I am submitting print outs of eStatements (Electronic bank statements) which I printed out at home logging into my account, got them stamped and signed on all of the pages by the Bank Customer Service agent. Would these be considered as Original

4.5 Give the start and end dates of the period for which you are claiming previous earnings
There are 4 entries here (like start date - end date)
I worked for 2 employers in past 12 months (4 months with 1 employer and 8 months with another ) . How am I supposed to enter these in this section
4.6 Earnings Table
Here there are 6 Entries only. I'm bit confused how these can be filled considering my case where 2 employers are to be mentioned.
