New SET[M] Form??

Does anyone know if there is a new SET[M] form as of this month or is it the same one as before?
I was sent a form last Feb. by the Appl. Forms Unit [ who no longer send out forms I discovered] and am wondering if I can still use it.

Have a look at this UKBA webpage.

In particular look at the right-hand column where there is a link to a new form, to be used as from yesterday.
