When I applied for my first UK Visa (T2 Gen) in 2017, I was living in Massachusetts, USA. However, my company employed an immigration solicitor that was located in the UK.

I assume the question is asking where *I* was applying from so I would enter Massachusetts, correct?

One additional confirmation, my Biometric Enrolment Appointment was in New York City. I'd still keep Massachusetts, USA in response to 'Where was it made?'. Correct?

I would say that refers to where you were located, Massachusets USA.
