WHV change to unmarried partners visa? help!

hi
can anyone help?

my partner is here on a working hol makers visa (canadian), Im british, her visa runs out early nov and we want to apply for her to stay.

Could someone please advise us on
*which forms we need to use
*is it worth spending the money for a same day appointment

what kind of evidence is best to take?

Does anyone have experience of this process they can let us know about.

We have been in a relationship since May 03 ...then my partner came to the UK for 3 months in July 03 as a visitor (returned to canada in oct 03 to get WHV), returned to the UK in nov 03 with visa and we have been together ever since.
