Wife's name while Birth Registration of Child

My wife is living in UK with her maiden name and surname on the passport and BRP.
However, she has taken my surname after marriage (her name remains same).
Our child born in UK has birth certificate with mother's married surname.

e.g.: Mother's maiden name: Suparna Suhas Koli => On passport
Mother's married name: Suparna Nishad Salvi => Child's birth certificate

My wife will be changing her name on passport to married surname in 2017 as her passport expires early 2018.

However, would there be any problem in getting our child's Indian registration and passport with my wife's married name on child's birth certificate? Do we have to make an affidavit stating both names belong to same person?

Well, if you you wife adopted your surname / family name then she should have applied for change of name in the Indian passport at that time!

That said,

IMHO, shouldn't be a problem if you have a marriage certificate showing your spouse's maiden name. Include a copy of the marriage certificate wherever your spouse's (or both parents') passport copy is required to be submitted - for the child's birth registration application or child's first Indian passport application.
