Work Permit + HSMP Combination - Need some URGENT Advice

Hello

I have completed 5 years of continuous stay - First 3 years on a work permit & the last 2 on a HSMP.

As evidence for work permit, what documents would I require?
The application form sates "you should provide details of the categories involved & the periods spent in each of them"

I have got all pervious payslips (from day 1) & have got P60's. Is that sufficient enough?

p60, p45, payslips(last three months), last three months bank statements, may need original wp or hsmp letters, life in uk cert and of course your application form.
