Correct form for Naturalization, Permanent resident evidenc?

Hi Everybody
Please can someone advice me on the correct form to naturalize, is it the form SET AN?
I am a non-EEA spouse of an EEA national resident in the UK and now I have a Permanent residence stamp.
My permanent resident stamp was given on 10/04/2014, but I have been resident in the country for more than 6 years on that day on the EEA route.
The form AN says that I have to prove evidence that my spouse I have been resident in the UK for 5 years when I become a permanent resident and 1 year as immigration free after that.
One more question - The last one year should I go by the immigration stamp for which I have to wait another year or can I prove that I am living for continuously 6 years in the country on the EEA route.
