Help with supporting document for Naturalisation

Hi,
I need some help making sense of the guidance, especially for which supporting documents to upload for my naturalisation application.
I am applying through permanent residence status; I received my permanent residence status on 16th March 2018, and I am an EEU citizen.
After going through the online process, I am asked to submit following documents :
The passport issued by Italy
Fine but do I have to scan every single page of the passport?
certificate proving you have a speaking and listening qualification in English at B1 CEFR or higher
all good here
Life in the UK Test pass notification
All good here too
Proof of living in the UK
Does it mean a rent agreement or the UK residency documentation or proof of working in the UK for the past 5 years? And if the last one, how many documents for each year?
Two referee declarations
All good here too
evidence of identity
Assuming the passport is ok here?
proof of Nationality
Again, assuming the passport is ok here?
