Please can you tell me about the forename... My forename is Tomasz (Polish one)
Do you think that I can request on the AN form to have the forename amended to read Thomas?
I read the guide and it is stated:

1.6 Enter your other names as you want them to appear on your certificate.

I too have the same question; as my husband wishes to add his proper family surname to the form AN. If anyone can help it would be really helpful

you may need to do it by deed poll, i am not sure if you can do it any other way.
