silly question about set(0) form page 4

Hi all
I am doing a final check on my form and documents before I post and want to make sure everything is right with the help of kind people from this forum.
I am applying for Ilr on the basis of long residence .
on top of page 4 where it says if you are applying as a HSMP migrant ,are you doing so under the terms of HSMP ilr JR policy ....yes or no,
as it doesn,t concern my application I should leave blank ,should not i?
second question on the same page ,payment details....Name on card...should i write my full name or write as it appears on the card i.e KK Gautam
I know it seems silly but thought it would be better to have your opinion.

I have written it down 'as it appears on the card' i.e. 'MR O A Syed'

please let the bank know that UKBA charging unit may take money from your account as application fees, a hassle that happens so often while paying through card

Thank you , i would visit the branch and inform them.
