ILR for Tier 1 dependant

Hello,
I recently got ILR visa, my wife is still on Tier 1 dependent. It would be great to get answers to below queries...
1. She got her tier 1 visa in April 2012, so she has to complete 2 years (not 5 years) before applying for ILR. is that correct?
2. Which form is used to apply for ILR for Tier 1 dependent? SET(O) or SET(M)?
3. Can this application be made 28 days before the 2 years qualifying period?
