Queries on ILR due next month (Jan 16)

Hi Members,

I've ILR due next month and have some queries. I'm currently on Tier 1 (General).

Dates:
Initial grant: 14 Jan 2011 - 14 Jan 2013
Entered in the UK: 24 Feb 2011 (90 day concession applies?)
1st extension valid till: 7 Feb 2016
2nd extension valid till: 18 May 2018 (I did this extension as the Tire 1 extensions were closed from April 2015)
No absence from UK

Just to confirm, I'm using SET(O) form for ILR application.

Queries:
Que 1: Am I right in thinking that I'm eligible to apply for ILR from 18 December 2015?

Que 2: I've used my degree for KoL (English language) for all applications, do I need to take KoL test?

Que 3: Do I need to show last 5 years address proof, if yes what documents are acceptable?

Dependents,
Initial grant: 28 Aug 2011 (outside UK)
Entered in the UK: 13 Sep 2011
Extended in Jan 2013
Current visa valid till : 7 Feb 2016
No absence from UK

Que 4: Are my dependents eligible for ILR?

Que 5: My wife has similar degree as mine, how can I check if that can be used for KoL (English knowledge)?

Que 6: I'm planning to apply for my ILR through premium service for my self and later by post for my dependents. Is this ok to do? If yes, which form shall I use?

Que 7: I guess I wouldn't have my new BRP of ILR by the time I apply by post for my dependents. Is this fine?

Que 8: What supporting documents for dependents shall I include?

Que 9: Do I need to pay healthcare charges for all of us?

I really appreciate your help on this.
